Location: Windham County
Health Care and Rehabilitation Services (HCRS) is looking for a compassionate and dedicated individual to join our team as a Shared Living Provider (SLP). In this rewarding role, you'll provide support to a 43-year-old male that would thrive best in a caring environment that provides a schedule and structure routine while encouraging independence. He would benefit from having a Shared Living Provider that takes him out to do things and keeps him busy.
Key Responsibilities:
- Transportation
- Medication Administration (full support)
- Hygiene reminders (bathing, clipping nails, laundry)
- Support managing funds and budgeting.
- Foster a loving, comfortable home environment.
- Meal preparation.
- Work collaboratively with a team of healthcare professionals, including case management, to ensure the best care and support.
About the Individual:
- Likes: This individual enjoys social interactions and is very friendly. He loves to tell good jokes and spend time with his peers. Some of his favorite activities include bowling, playing wiffle ball, listening to music, watching tv/movies, and he has expressed interest in learning how to cook. He would also like to learn karate. This participant likes to be laid back and just enjoys life.
- Support Needs: This individual requires support with reminders to keep up with his personal hygiene and where clean clothes. Assistance with laundry is needed as well. He is working towards being more independent throughout the day but will still require supervision and guidance. This participant currently works Thursday afternoons and transportation is provided by the employment team. The client needs to be taken to movement therapy at the HCRS Springfield office once a week.
