Job Title: Family Case Manager
Reports to: Director of Programs
Hourly Rate: $24.76
Location: Lawrence, MA
Overview:
Family Case Manager helps families and individuals establish and work on short term goals to help them stabilize their lives and move toward securing more stable housing. Provide support services such as: information and referral, goal setting, problem solving and linkages to any needed wrap around services.
Essential Duties & Responsibilities
- Complete an initial family assessment, identifying both needs and strengths and acts upon critical needs appropriately and immediately
- Orients clients to the shelter programs, requirements and consequences
- Resolving or mitigating tenancy-screening barriers such as securing ID
- Assistance enrolling in entitlement benefits such as (SNAP, TAFDC, SSI)
- Working with parents to address educational and health needs of children.
- Connect families with community resource.
- Create a model to recruit and maintain strong relationships with landlords
- Develop a family care plan with short- and long-term goals.
- Provide direct family counseling and referrals.
- Assist families in securing and accessing appropriate entitlements, benefits and resources.
- Evaluate family progress.
- Coordinate monthly house meetings with residents.
- Coordination/support resident supervisors/staff.
- Collaborate with other organizations in support of resident families.
- Maintain client files to include conversations, warnings, progress towards goals and documentation of any incidents.
- Report critical incidents immediately through no-compliance form. CNV 2
- On occasion may be called to provide progress reports to the Board of Directors
- Supervise and support program staff (staff meetings, direct observation, written evaluations)
- Evaluates work performance of staff as specified by agency personnel policies.
- Perform other duties which may be assigned from time to time
Additional Essential Functions
Sitting, standing, reaching above, stooping, bending, pushing, pulling, traversing, thinking, speaking, writing, concentrating, interaction with others. Must be able to lift up to 25lbs.
