Job Title: Case Manager - Greater Boston Tenant Base (GBTB) Supportive Housing

Overview:
The Case Manager provides case management to formerly homeless individuals and families. This Case Manager supports clients in remaining stably housed in apartments in a scattered site model; or in congregate locations that are community based Using evidence based best practices like Motivational Interviewing and a collaborative approach, the Case Manager works with clients to develop and monitor individual service plans (ISP) which include educational, vocational, medical, life skills and housing goals. The Case Manager will also assist program participants to obtain resources and services as requested and/or needed to address barriers to housing as stated above including but not limited to CORI mitigation, credit repair, and assistance with financial applications.
Responsibilities / Essential Job Functions:
- Work with clients to develop Individual Service Plans (ISP) which include financial, mental health, physical health, legal, educational, vocational, and life skills goals
Meet with clients regularly to review ISP progress and plan next steps. - Make referrals to community programs and assists clients in obtaining resources and services for areas on the client’s ISP and as needed to address barriers to independent living.
- Develop partnerships and serve as a liaison with community based organizations, and attend meetings with such organizations as needed;
May assist clients in finding housing options;
Assist client with move-in transition supports, including but not limited to: furniture and furnishings assistance, budget support, and other resources and services as needed;
Document all interactions with clients utilizing appropriate reporting mechanisms, including our Homeless Management Information System/ web-based data collection system as well as internal record keeping software (Salesforce) - Attend staff meeting, supervision, and trainings as required;
- Attend all applicable trainings, including Motivational Interviewing, Trauma Informed Care, and Harm Reduction Approaches, and puts the skills and techniques into practice with clients
- Complete reports for program as needed;
- Perform other tasks as requested by the Program Manager, Director of Supportive Housing, or the;Senior Dir. Of Housing and Clinical Supports
- Observe all agency rules and policies

Work Environment
- Cubicle space within large office building; prolonged sitting at a desk and working on a computer, OR, requires equal amounts of sitting and standing, must be able to climb stairs. Office is open to the public and visitors only by appointment. Position requires a minimum of four days onsite. Office is climate controlled with heat and air conditioning.
- Must be able to occasionally ascend/descend stairs up to three or more sets, frequently move equipment weighing up to 25 lbs., must be able to remain in a stationary position 50% of the time.
