POSITION SUMMARY:
All Together Recovery LLC (ATR), provides multiple levels of ASAM SUD/BHSO care, including recovery housing, education, job training, employment, and other recovery related extensions to the family in for-profit and non-profit environments. The Targeted Case Manager (TCM) will carry a caseload of no more than twenty-five clients. TCM will provide quality case management, support, advocacy, community resources, ongoing referral and assessment for services provided and/or needed for each client. The TCM will perform case management services as outlined by the state and in accordance with state and federal laws, regulations, and company policy.
GENERAL RESPONSIBILITIES AND REQUIRMENTS:
- Must have a valid Kentucky driver’s license.
- Participate in all necessary staff meetings.
- Be proficient in all ATR rules, policies, and procedures and be able to educate clients in them.
- Model servant leadership.
- Operate with integrity in all circumstances.
- Communicate effectively and be responsive to verbal and written correspondences.
- Must meet at a minimum of four contacts monthly, at least two face-to-face with client or five per month for clients with a dual diagnosis and maintain a client load of twenty-five.
- Review treatment goals and objectives and assist clinician with developing crisis prevention plan, and strength-based needs assessments with client and family and document as required by state and office regulations.
- Complete contact notes within 48 hours.
- Ensure legal documentation is obtained prior to placement or service delivery.
- Must review treatment goals and objectives quarterly and make necessary changes after a team meeting.
- Obtain current knowledge of local resources in the client’s county of residence.
- Understand the client’s diagnosis and the challenges it presents to their family and provide information, support and/or intervention to the client and their family.
- Meet with other service providers to plan and coordinate treatment to ensure the integration of the treatment across the client’s home, school, and community and to achieve the desired outcomes and goals identified in the treatment plan.
- Participate in continuous improvement programs and services for the organizations.
- Demonstrate ethical problem and conflict resolution skills.
- Present and always perform in a professional manner.
- Facilitate interdisciplinary treatment team meetings when needed.
- Provide direct care for individuals, families, and groups which will include linking the identified resources, monitoring services received, advocating for client needs, discharge planning and follow up. Will also provide support to the client’s family members if applicable.
- Document, review and update treatment plans, as necessary.
- Ensure that all four contacts are made within the required time frames with a minimum of two contacts being direct face to face contacts.
- Submit all required documentation and maintain client charts as required by state and federal regulations and company policy.
- Able to work in a team environment and independently.
- Ability to follow through with all tasks to completion.
- Work hours may vary based on clients/family schedule.
- Travel may be required and must have your own reliable transportation.
Other duties as assigned by clinical program manager.
BACKGROUND/EDUCATION:
- Bachelor of arts or science in a human services or behavioral science field with one year post educational work experience working with adults in a human service setting.
- Successfully complete a company approved TCM training within 6 weeks of employment as a TCM.
- Maintain continuing education credits to remain in compliance with TCM standards.
- Attend ongoing clinical supervision.
- Ability to work in a fast paced, dynamic environment.
Salary : $16 - $18
