Key Responsibilities :
- Conduct initial and ongoing assessments to determine client needs and strengths.
- Develop and implement individualized case management plans to address medical, mental health, housing, employment, and social service needs.
- Assist clients in accessing community resources , including healthcare, housing, legal assistance, employment, and educational services.
- Coordinate care with therapists, medical providers, probation officers, and other stakeholders to ensure a holistic approach to recovery.
- Advocate for clients to receive necessary services while promoting self-sufficiency and empowerment .
- Monitor client progress, update service plans as needed, and provide crisis intervention when necessary.
- Maintain accurate and timely documentation in compliance with state, federal, and facility guidelines.
- Ensure compliance with Kentucky Medicaid and Targeted Case Management (TCM) regulations .
- Participate in team meetings, case consultations, and staff training to enhance service coordination.
- Bachelor’s degree in social work, psychology, counseling, or a related field (Master’s preferred).
- Must meet Kentucky state requirements for Targeted Case Management certification or be eligible to obtain certification.
- Experience working with individuals with substance use disorders and co-occurring mental health conditions.
- Knowledge of trauma-informed care, harm reduction, and recovery-oriented practices .
- Strong understanding of community resources, Medicaid services, and social service systems in Kentucky .
- Excellent communication, documentation, and organizational skills .
- Ability to work independently and as part of a multidisciplinary team .
- Valid Kentucky driver’s license and reliable transportation for client appointments.
- CPR / BLS certification (or willingness to obtain).
- Certification or training in Motivational Interviewing, SBIRT, or other evidence-based interventions .
- Experience with electronic health record (EHR) systems and Medicaid billing .
- Familiarity with 12-step programs, peer support, and recovery-based services .
