HHCI is in search of a Family Specialist (FS) for its Assertive Community Treatment (ACT) Team. This position is currently remote and field-based, but will require the FS to report to the office (located in Baltimore, MD) as needed. Remote work capabilities are available for as long as tele-health services are permitted by HHCI’s regulators and funding sources. The need to report to the office will be determined and communicated to the FS by the Clinical Director (CD)/CEO.
Position Summary:
The Assertive Community Treatment (ACT) team FS provides ACT services to a mutually shared caseload of ACT clients within HHCI’s treatment and recovery services division. The FS is responsible for utilizing evidence-based practices to assist their clients with meeting their treatment and recovery goals, and operating within the scope of their profession. This includes providing client psychoeducation, family/natural support psychoeducation, motivational interviewing, cognitive behavioral techniques, dialectical behavior techniques, and trauma-informed care.
Responsibilities:
- Provide ACT services to each admitted client of the ACT team at least monthly
- Conduct psychosocial assessments within 30 days of admission for newly admitted clients, and annually thereafter for existing clients
- Conduct additional mental health assessments as needed and directed by the CD
- Coordinate referrals for additional support for admitted clients
- Accompany clients to appointments, if needed
- Enter, sign, and submit all electronic clinical documentation to the CD within 24 hours of the encounter with the client, including progress notes and assessments
- Conduct intake screenings for prospective clients
- Prepare for, attend, and participate in daily team meetings
- Engage in person-centered treatment planning with clients
- Conduct requested self-audits of charts for assigned clients, as well as unplanned chart audits
- Collaborate with consented external providers and internal personnel regarding the treatment goals of mutually shared clients
- Involve consented collateral contacts into the client’s treatment and care, including offering family therapy/counseling as needed and permitted by the client
- Complete required trainings within established timeframes
- Rotate the 24-hour on-call ACT phone with other members of the team
- Comply with all relevant standards and regulations of HHCI, the state of Maryland’s Behavioral Health Administration (BHA), The Joint Commission, HIPAA
- Perform other duties as requested by the CD
Required Qualifications:
- Master’s Degree in Social Work (MSW), Mental Health Counseling (MHC), Marriage and Family Therapy (MFT), Creative Arts Therapy (CAT), Counseling Psychology, Clinical Professional Counseling, or a related field from an accredited master’s program
- Licensure in the state of Maryland as a: LMSW, LGPC, LGMFT, LGPAT, LGADC, LCSW, LCSW-C, LCPC, LCMFT, LCPAT, or LCADC
- A minimum of 1 year of providing evidence-based family counseling/therapy within the mental/behavioral health sector
- Proficiency in Microsoft Suite (Word, Excel, Outlook, Power Point, Access), electronic health records, and the use of computers/laptops
Preferred Qualifications:
- Strong preference for experience working with a multi-disciplinary mobile team
Job Type: Contract
Pay: $80,000.00 - $100,000.00 per year
