Job Details:
This is a long-term care travel position in Liberty, New York. The successful candidate will have a strong background in providing patient-centered care and be able to work effectively in a fast-paced environment.
Responsibilities:
- Deliver high-quality patient care to residents in a long-term care setting
- Develop and implement individualized care plans with the healthcare team
- Monitor patients' health status and report any changes to the healthcare team
- Maintain accurate and detailed records of patient interactions and treatments
Requirements:
- Current LPN/LVN licensure in New York
- At least one year of experience working in long-term care settings
- Ability to work 12-hour night shifts
- Excellent communication and teamwork skills
