Social Care Navigator Role
We are seeking an energetic and passionate individual to support our mission as a Social Care Navigator. This non-clinical role will involve conducting health-related social needs screenings, referrals to appropriate services, and follow-up with clients. The successful candidate will have excellent communication skills, be able to work effectively in a team environment, and have a strong understanding of community resources.
Key Responsibilities
- Conduct screenings and interviews with Medicaid members to identify their social care needs.
- Verify demographic information and eligibility for services using various documentation platforms.
- Establish resource needs and connect clients to relevant health and social care services.
- Develop care plans based on client needs and provide ongoing support to ensure effective service delivery.
Qualifications and Experience
- Bachelor's degree or equivalent required; 2-3 years of relevant work experience preferred.
- Experience working in community healthcare settings, health coaching, or patient navigation is highly desirable.
- Valid driver's license and reliable transportation required; bilingual skills a plus.
