Job Title: Care Navigator - Health Education Support
Duties:
- Assist patients in navigating medical services, ensuring timely attendance at appointments.
- Address medication adherence and provide coaching to overcome challenges.
- Provide health education tailored to specific populations as outlined by program requirements.
- Collaborate with medical providers to facilitate patient compliance with care plans.
- Manage a caseload of up to 20 patients, conducting health education sessions as needed.
- Reassess patient entitlements and address logistical barriers, such as transportation or childcare.
- Maintain comprehensive documentation and support data entry for grant compliance.
- Perform additional responsibilities as required by the program.
Skills Required:
- Effective communication skills, with preference for bilingual (Spanish-English) candidates.
- Strong attention to detail and organizational skills.
Pay Range:
- Weekly: $1,300 - $1,550
