Overview
As a critical member of an interdisciplinary team, the Case Manager provides case management services to adult, pediatric and geriatric clients who are receiving medical or behavioral health services. The Case Manager is pivotal in addressing social determinants of health by assessing the client’s needs and connecting them to appropriate services.
Key Role Responsibilities
- Provides case management assessments, treatment planning and interventions. Engages clients and builds relationships by greeting clients with a warm introduction, communicating with empathy and compassion, and working on goals identified by the client.
- Collaborates with other providers in an integrated, multidisciplinary care team, attending interdisciplinary meetings as required. Engages other team members in problem solving regarding client care, and links clients to ongoing medical, behavioral health, and supportive services.
- Assertively and systematically identifies clients who are eligible for financial, housing and other benefits. Assists clients in applying for financial, housing and other benefits, including completing Coordinated Access applications, other housing applications, disability benefit applications, or employment related services. Identifies and supports the client’s use of appropriate external resources.
- Provides referrals to crisis intervention services such as emergency shelter and food assistance.
- Completes documentation of services in a thorough manner in accordance with required timeframes within the client’s electronic health record. Documentation includes but is not limited to : initial assessments, treatment plans, progress notes, and discharge summaries.
- Effectively supports the client’s case management plan by escorting the client to appointments and visiting clients in their home or in the hospital as needed.
