Patient Care Coordinator
HCPSC is a full-service, privately held provider of community-based outpatient health services. We are a rapidly growing provider of outpatient health services in the Conway, SC area. We are looking for a Patient Care Coordinator to join our team and provide quality care to our patients. The ideal candidate will have a strong interest in community work.
Responsibilities:
- Responsible for greeting patients, scheduling patient appointments, ensuring all patient paperwork is properly completed including demographic and HIPAA forms.
- Applies knowledge of funding resources, services, and clinical standards to everyday tasks.
- Assists in determining the financial status of patients and their eligibility for health services, assists patients in accurately completing appropriate forms and documents for the required information.
- Educates patients and family members about health conditions, medications, and instructions.
- Develops care plans with care team to ensure patients’ safety.
- Provides medical information to financial services for maximum health facility and patient reimbursements.
- Obtains authorizations from insurance companies when necessary and enter in all patient information and financial information in the computer system in a timely manner.
- Demonstrates the ability to establish and maintain effective, productive relations with co-workers, supervisors, patients, and the public.
- Maintains a neat/well organized work area. Responsible for maintaining an adequate number of supplies.
- Always maintains patient confidentiality.
- Performs other duties, as assigned.
Requirements
- High School Diploma/GED
- Associate's degree preferred
- 2-3 years’ experience in Customer Service-related roles.
- Bi-Lingual is a plus
- Must be able to work independently with minimal supervision.
- Must be able to work in a fast-paced environment with multiple tasks and deadlines.
- Previous experience in outpatient mental health preferred.
- Experience with EHR systems preferred.
Job Type: Full-time
Expected hours: 40 per week
