Job Locations
- Handling patient calls such as medication reconciliation and adherence for a variety of clients and programs to meet performance and quality standards and program goals using critical thinking skills.
- Engage patients through motivational interviewing to assist patients in changing behaviors related to medication use and adherence.
- Accurately complete all electronic documentation to ensure compliance with company standards, program goals, guidelines and government requirements
- Collaborate with healthcare providers on patients behalf to address drug therapy problems
- Participate in required trainings as assigned and support security and compliance related activities and task throughout execution of all job duties
