We’ve got a great opportunity for a Clinician to join the Community Access Recovery and Engagement (CARE) Team at 41 Montebello Rd in Pueblo, CO.
What You Would Be Doing
As the Clinician for the CARE Team, you would be providing collaboration to other team members and community-based direct care services to clients according to the DACTS Fidelity model to ensure that our clients receive the best care possible. You would attend to clients’ needs as you conduct assessments, plan treatment development and reviews, and provide individual and group therapy. You would be collaborating with the program director to offer efficient and effective evidence-based treatment services. You would represent Health Solutions among our community agency partners (e.g., probation, parole, social services) to ensure that we are doing all we can to support our clients.
Specifically, the Clinician:
- Provides exceptional clinical care to our clients in the community;
- Conducts substance abuse, mental health, and co-occurring assessments;
- Carries a small caseload and assists with emergency clinical care;
- Is an active team member in providing comprehensive care according to the DACTS Fidelity Model;
- Facilitates individual and group therapies;
- Provides case management services;
- Coordinates enrollments and discharge of clients;
- Maintains accurate and up-to-date written and electronic records;
- Participates in collaborative treatment planning with clients;
- Collaborates with community agencies and partners;
- Participates in planning, developing, and implementing the DACTS Fidelity model;
- Performs other job-related responsibilities as assigned.
Physical requirements include the following:
- Frequently stays stationary for prolonged periods of time.
- Frequently moves within and between facilities.
- Frequently exposed to stressful or emergent situations.
- Frequently operates a computer and other office equipment to record health data.
- Constantly communicates with clients and others.
- Occasionally moves equipment and/or materials up to 25 pounds.
- Frequently drives between facilities or for other assigned duties.

Qualifications
What We Are Looking For—The Must-Haves
- Master’s degree in social work, psychology, or a closely related field
- Experience working successfully with a multi-disciplinary team
- Computer proficiency
- Valid Colorado driver’s license and access to transportation during work hours
What We Would Like to See in You—The Nice-to-Haves
- Spanish language skills
- Colorado CAC II, III or LAC
- Colorado clinical license (i.e., LPC, LCSW, LMFT)

Salary : $60,000 - $76,458
