Position: Care Manager (LCSW) – Care Management Team
Location: Torrance, CA
Schedule: Full-time, 8-hour Day shifts
The Care Manager (LCSW) will be part of the Care Management Team within the Department of Psychiatry. This role involves collaborative practice with patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners, and the community.
Social workers in this department facilitate communication and care coordination across a continuum through effective transitional care management. The role acknowledges the patient’s right to self-determination, the impact of social determinants of health, and the complexities of care (including barriers to coping, abuse, mental health, substance use, lack of resources, and psychosocial issues).
Required Qualifications- Master's Degree in Social Work.
- Coursework/Training – All LCSWs must follow the guidelines set by the Los Angeles County Department of Mental Health for LPS designation and maintain their designation in current status.
- California Clinical Social Worker License (upon hire).
- 2 years of experience in a healthcare setting working with a psychiatric mental health patient population, preferably in an inpatient setting.
- Computer proficiency.
- Behavioral Health Clinician designation to perform 5150 evaluations within 18 months of hire.
- CPI certification (nonviolent crisis intervention training) within 1 year of hire.
- Discharge planning experience.
