JOB SUMMARY
Under the direct supervision of the Program Manager, the Case Manager is responsible for offering technical assistance and case management linkage and referrals as needed. Additionally, the Case Manager coordinates with the treatment team the plan of care for participants with mental illness and/or substance use issues. The Case Manager acts as a liaison between participant/family and clinician to ensure that their non-clinical case management needs are being met.
ESSENTIAL DUTIES
- Creates robust linkages with community organizations who provide services to youth and young adults, including schools, community centers, youth social services agencies, etc. to identify successful referrals
- Builds referral partnerships with the Illinois Juvenile Court System and youth probation to ensure successful referrals care coordination
- Manages a caseload of youth and young adult participants with complex needs to ensure adequate services are provided to assist participants with meeting their treatment needs.
- Links participants to needed and available community resources including legal, medical, housing, transportation, mental health, financial, etc.
- Provide outreach and case management services including intakes, assessments, SDOH forms, basic service planning, referral and linkage management, and advocacy to and for individuals who are homeless, experiencing SMI and/or SUD, or are justice impacted. Supporting participants in obtaining government benefits such as Medicaid, SNAP, etc.
- Provides ongoing supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the service/treatment plan in order to help the participants to achieve the stated goals and objectives.
- Provide crisis management services as needed to caseload, including for mental health crises, family issues, and other types of client-identified concerns.
- Documents all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintain the case records in accordance with agency and regulatory standards and requirements.
- Provides anti-stigma education to the community and individuals dealing with mental illness and/or substance use disorders as well as information on prevention, treatment, and recovery for participants.
- Meets regularly with the Program Manager as a means of enhancing professional growth, reviewing and processing the provision of case management services, and dealing with appropriate administrative issues.
- Maintains a networking relationship with community agencies, clinics and supportive services for referrals/linkages and ancillary services.
- Provides specific case management support, services, and documentation associated with any private or public grants.
- Performs other duties as assigned by the Program Manager.
REQUIRED SKILLS/ABILITIES
- Work will require traveling and will require the candidate to be onsite.
- Strong organizational, communication, and critical thinking skills, including the ability to work as part of a collaborative treatment team.
- Basic knowledge of computer keyboard.
- Bilingual/Bicultural is preferred (English/Spanish).
- Must be flexible with hours to accommodate participant’s needs.
- Possession of valid Illinois Driver’s license and willingness to use personal vehicle in course of employment, including transportation of participants.
- 100/300/100 Automobile Insurance.
- H.A.S. requires all staff to work onsite.
EDUCATION & EXPERIENCE
- GED/High School Diploma.
- Have two years of case management experience in a non-profit setting.
- Knowledge of youth and / or criminal justice services in Chicago is strongly preferred.
PHYSICAL REQUIREMENTS
Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Repetitive motion. Substantial movements (motions) of the wrists, hands, and/or fingers. The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading.
