Now Hiring – Intake Coordinator Hours: 8:30 AM – 5:00 PM
The Intake Coordinator is responsible for managing patient referrals and ensuring a smooth admission process. This role involves working closely with patients, families, and healthcare professionals to coordinate care and maintain accurate records.
Key Responsibilities:
- Complete initial inquiry forms for all potential admissions and forward them to the appropriate Hospice Nurse, Director of Clinical Services, and/or Administrator.
- Ensure that all possible referrals are converted to admissions whenever feasible.
- Notify the Administrator if a referral does not result in an admission.
- Track and report daily inquiries and referrals, providing a weekly summary to the Administrator.
- Assist patients, families, and responsible parties with completing the admissions packet as needed.
- Work collaboratively with staff to ensure that all written or verbal consents are obtained.
Qualifications:
Education:
- Associate's degree in a related field preferred.
Experience:
- 2 years of experience in a related capacity preferred.
- Must have a valid driver's license and a reliable source of transportation.
- Willingness to travel as needed.
- Excellent communication, interpersonal, and organizational skills.
