Value Based Care Model
- Physician: As the ‘quarterback’ or ‘captain’ of the team, physician will deliver holistic care to patients working in a coordinated manner with a care team comprised of a clinical pharmacist, nurse, health coach, behavioral health specialist, and medical assistant.
- Clinical experience in family medicine, internal medicine, or ambulatory care setting is preferred.
- MD/DO Board Certified/ Board Eligible (Prefers Doc earlier in their career)
- Schedule 8-5:30 M-F (have been flexible with 4x10hr shifts. Last Appt. ends at 4:40p
- EMR: NextGen
- NO Call
- Ea. visit 30 minutes Avg. -Annual Wellness visits 1hr 12-15 patients a day/ Complex patients seen 6-8x’s a year
- Patient Demographic: Mostly Seniors. Some Peds.
- Independent Primary care center not affiliated with the hospital
- Uses With Anthem PicassoMD for referrals
- Partner w/LabCorp
- Urgent Care Center next door
- Pay Range: $230-$250k Base 10% Bonus
- Full benefits, Free Urgent Care, 401k, 1wk. CME 4wks. PTO 6 paid holidays
- Board Certified/Board Eligible in: FM, IM or Geriatric Medicine
- Open to Residents and Fellows
- Open to Visa Candidates
- Basic Life Support (BLS) Certification
- Advanced Cardiovascular Life Support (ACLS)
- National Institutes of Health (NIH)
- Sound knowledge of anatomy, physiology, pathology, and medical terminology.
- Active unencumbered physician New Hampshire state license
- Graduate from a fully accredited MD or DO program
- One year of work experience as Family Practice Physician
- One year of EMR experience
- Board Certified
- Maintains and updates clinical knowledge and skills.
- Participates in relevant continuing education programs.
- Knowledge of and dedication to the highest patient care and provide the utmost service excellence to all patients.
- Ability to take initiative and to exercise independent judgment, decision-making and problem-solving expertise.
- A can-do, team oriented attitude is essential, as the ability to work independently and to interact easily with physicians, other managers and staff throughout the department and hospital.
- Effective interpersonal/communication, organizational, and the flexibility to handle multiple tasks.

Salary : $230,000 - $250,000
