Case Manager - LVN / LSW
HealthCare Support is actively seeking a Case Manager - LVN / LSW to fill an opening in Fort Stockton, TX with a managed care organization.
Daily Responsibilities for Case Manager - LVN / LSW:
- Determine needs and eligibility of members for various services through field-based collaboration with medical, behavioral, and social support staff.
- Conduct screening and risk assessment interventions per program guidelines.
- LTSS service care manager will have 3 days of travel for visits seeing the members in their home and 2 days working remote for documentation of assessments as well as any follow up and tasks required to work.
- Location: Remote/home field visits in Fort Stockton and surrounding areas (Travel up to 75% of the time with visits up to 2 hours away each way)
- Shift: Mon-Fri, 8am-5pm CST
Required Qualifications for Case Manager - LVN / LSW:
- 5 years of experience working with the aging, blind, and disabled population
- Clear and active Texas LVN or LSW license
- Good job tenure
- Bachelor’s degree in Social Work, Health Services, Behavioral Science or related field or HS diploma or GED ok if LVN
- Microsoft Office, excel and reliable internet
- Clear and active driver’s license, reliable transportation, and insurance

Pay Details: $25-27/hr, depending on licensure and experience
