Daily Responsibilities for the Field Supervisor:
- Delegate responsibilities and supervise personnel in collaboration with the Care Delivery Manager to ensure quality care and compliance. Evaluate participants' health needs considering physical, mental, and environmental factors, and guide home health staff in delivering services. Collaborate with the interdisciplinary team to develop and maintain comprehensive care plans and ensure all documentation and regulations are met while maintaining positive relationships and staying current with health practices
- Shift
- Monday - Friday 8:30 am - 5:30 pm | No on Call
Required Qualifications for the Field Supervisor:
- Clear and active NY State RN License
- 5 years as an RN
- 1 years of recent supervisor experience
- 1 years of Home Health
