HealthCare Support is actively seeking a Community Health Service Worker to fill an opening with a Managed Care Organization in Macon, GA .

Daily Responsibilities for Community Health Service Worker :

• Training will be in Atlanta for first week and client will reimburse travel, room, and board
• Responsible for conducting home visits to high-risk members to complete a non-medical assessment to identify barriers impeding care such as home safety issues, transportation and other social determinants of health.
• Connecting with community partners and / or organizations to build partnerships that help to address the needs of our members.
• Coordinate, implement and participate in community events within the designated region but could include the entire Georgia market.
• 80% of the role will be remote
• This will be Contract to Permanent role, schedule is Monday-Thursday, 9am-6pm and Friday, 8 : 30am - 5 : 30pm (may have some OT and weekends, need to be flexible)
