Position Overview:
As a CFSS Consultation Specialist, you will be responsible for scheduling intake calls and service delivery plan (SDP) calls, educating clients about the MN DHS program, completing SDP documentation, and ensuring timely completion and signatures. Your role will be pivotal in ensuring clients understand and access the support they need for independent living.
Key Responsibilities:
- Schedule intake calls and service delivery plan calls with clients.
- Educate clients on the Minnesota Department of Human Services (MN DHS) program. Complete service delivery plan documents and ensure the timely completion and signature of these documents.
- Provide excellent customer service and support to clients throughout the process.
- Collaborate with other team members to ensure quality care and compliance with program requirements.
Qualifications
- Previous experience in customer service or healthcare-related roles, preferably in personal care assistance or a similar field.
- Strong phone experience and ability to communicate effectively with clients.
- Knowledge of or willingness to learn about MN DHS programs (training will be provided).
- Strong organizational skills and attention to detail.
Starting pay for CFSS Consultation Specialist: $19-$22/hr
Care Coordinator/Front Desk duties include:
source new hiring avenues, develop long-term recruiting strategies, host, and participate in community hiring events and job fairs, actively work toward hiring diverse and qualified caregivers, onboard new hires promptly and accurately, work closely with Care Coordinators to staff clients with caregivers, effectively mediate caregiver-client issues, supervising caregivers work schedules, and providing training/coaching to caregivers.
Working directly with clients, assisting them with paperwork and managing their services and approved hours.
Building relationships with clients, their families, support teams and case managers.
This is an in-office opportunity to work independently and with other office staff as a team in our newest location in St Louis Park
Includes general office work and administrative duties such as answering phones, greeting walk-ins, data entry, scanning documents, light cleaning and special projects as needed.
Care Coordinator/Front Desk starting pay: $18-$21/hr
