Job Responsibilities
This position requires a Registered Nurse with experience in hospice care to provide direct patient care, manage pain and symptoms, and coordinate care with interdisciplinary teams.
- Develop and implement individualized plans of care to meet patient needs
- Collaborate with physicians and other healthcare professionals to ensure seamless coordination of care
- Communicate effectively with patients, families, and caregivers to address concerns and promote understanding
