Position Summary
Healthonomic Primary Care is seeking a dedicated Primary Care Physician to join its team full-time. The ideal candidate will provide high-quality patient-centered care, adhere to clinical and ethical standards, and collaborate with staff in a fast-paced, community-focused outpatient setting.
Key Responsibilities
- Diagnose, treat, and manage acute and chronic conditions in a diverse patient population.
- Maintain an average daily patient volume of 20β25 patients.
- The work schedule is 5 days per week, including one evening weekly and every other Saturday for the first six months (monthly thereafter).
- Participate in weekend on-call rotation (approx. once/month).
- Supervise and collaborate with Advanced Practice Providers (NPs, PAs).
- Maintain timely documentation and ensure compliance with regulatory requirements, HIPAA, and internal policies.
- Engage in promotional and community health events as required.
- Adhere to quality improvement programs and participate in periodic internal audits.
- Complete CME activities and maintain board certification and licensure.
Required Qualifications
- M.D. or D.O. degree with board certification (or eligibility) in Internal Medicine or Family Medicine.
- Active and unrestricted Illinois medical license and DEA registration.
- Eligible for participation in Medicare, Medicaid, and major insurance plans.
- Must meet credentialing standards for Advocate Physician Partners and affiliated hospitals.
