JOB DESCRIPTION
MINIMUM QUALIFICATIONS:
- Education, Experience or Equivalent Combination:
o Bachelor’s degree preferably with a major in Community or Public Health, Health Education, Nutrition, Communications, or related field.
o 5 years progressive experience leading community health programs, communications and partnerships
o Minimum 2 years of experience in community or health program planning and delivery
- Licensure/ Registration/ Certification:
o Valid driver’s license with access to personal vehicle for driving between office and community locations throughout the metro area.
- Knowledge, Skills, and Abilities:
o Deep knowledge of community and public health needs assessment, program develop, communications and evaluation, including community benefit.
o 111046Excellent written and verbal communication skills including presentations and health communications for a variety of target audiences.
o Strong relationship building skills with community partners.
o Strong interpersonal and communication skills
o Ability to thrive in a dynamic, collaborative, and complex environment, managing multiple priorities and partners.
o Superior organizational skills with ability to organize large amounts of supporting materials, prioritize and meet multiple deadlines.
o Proficient in Microsoft Word, Excel and PowerPoint
o Cultural competency & a comprehensive understanding of issues of health equity, diversity and inclusion.
o Ability to lift 20pounds and be physically active loading, unloading and delivering materials to community partners and assisting with event set up and tear down.
PREFERRED QUALIFICATIONS:
- Education, Experience or Equivalent Combination:
o Advanced education such as MPH, MS or Registered Dietitian highly desirable
o Previous leadership or supervisory experience
o Proficient with data in Excel spreadsheets and graphing and data visualization.
o Experience with health promotion program evaluation
ESSENTIAL DUTIES:
1. (35%) – Fosters and manages the community relations, partnerships and collaboration to advance progress with community health priorities including:
o Works closely with the VP of Community Health and Partnerships, local hospital leadership, Communications/Marketing, Foundation leaders, care system, health plan, institute and other internal stakeholders.
o Builds trusted relationships with community partners including schools, business, public health, government agencies, nonprofit organization and other community partners.
o Communicates goals and timelines and provides work direction to team members, consulting across the team and with VP of Community Health and Partnerships as needed.
2. (30%) – Develops and manages detailed project plans, timetables, resources, tracking and communications to meet all deadlines.
o Manages community relations and community health activities including:
§ Wilbur Thomas Community Service Awards
§ Community Benefit activities information gathering and narratives, in partnership with finance.
§ System-wide Community Health Needs Assessment process, in partnership with CESR
§ Sponsorship recommendations and processes for communications and budget tracking
§ Volunteerism strategies for colleagues and external volunteers.
§ Support for charitable giving activities including the One Campaign and partnerships with sales and annual engagement survey.
o Manages program and sponsorship budgets, including Foundation reimbursements, grant and other revenue, ensuring excellent stewardship of resources from multiple sources.
o Ensures timely development of communication plans for websites, social media, newsletter, email and community channels, working closely with Communications/Marketing.
o Communicates Community Health and partnership plans, goals and results to internal and external stakeholders and partners.
o Develops and utilizes appropriate evaluation metrics and tools, based on best-community practices and communicate data and results using dashboard measures for community initiatives using data visualization tools.
o Partners with HealthPartners institute, Informatics and internal and external researchers on evaluation projects and publications.
o Develops and submit grant applications and funding request as appropriate.
o Serves as a key contact for community relations.
o Represents HealthPartners on community collaborations with shared goals.
3. (5%) - Contributes to the total effectiveness of the Community Health and Partnerships team by setting communicating openly, solving problems proactively, offering creative ideas and working as a positive, engaged team member.
4. (5%) - Represents Lakeview and the HealthPartners family of organizations.
