North County Serenity House, A Program of HealthRIGHT 360 was founded in 1966 to provide substance use disorder services in the community.  North County Serenity House provides a gender-responsive and trauma-informed environment, using evidence-based and best practices that recognize and account for the role that trauma frequently plays in substance use and criminal histories of women. For clients with co-occurring mental illness, we provide integrated substance use and mental health services which treat both conditions as primary. Our residential facility serves up to 120 women (with capacity for up to 20 children under 5 years of age) seeking recovery from substance use disorders.

Child Welfare Services (CWS) Care Coordinators are responsible for assessing client and children needs and strengths in support of family health and recovery. Through case management in individual and group settings, delivering curricula in accordance with program specific requirements, CWS Care Coordinators assist clients with children in navigating systems of care while providing a supportive
treatment environment.

         

Key Responsibilities

    

Individual Treatment Responsibilities: Facilitates individual case management sessions with each caseload client who is involved with Child Welfare Services or who has a child under the age of 6 and ensures that each mother and child is referred to Jumpstart program upon entering treatment. Attends Jumpstart Classes and Services as needed that are provided onsite by Healthy Developmental Services.

Keeps consistent contact with Health Developmental Specialist. Communicates weekly with Rady’s DSEP to track developmental screenings of children. Provides learning experience opportunities and offers clinical support to assist clients in meeting their treatment goals. Proactively links clients to both internal and external resources based on their treatment needs and follows up on the progress/status.

Facilitates monthly case conferences for clients who are involved with Child Welfare Services which include all parties involved in client’s case. Attends Team Decision Making Meetings when invited by Child Welfare Services and maintains regular contact with CWS workers to allow for advocacy for each client.

Treatment Setting Responsibilities: Reports to other referring agencies weekly. Performs crisis intervention and communicates with treatment team as unforeseen situations arise. Documents client updates, incidents, changes in visitations and placement in the facility log daily. Attends required trainings and meetings. Plans and facilitates client celebrations and special events. May work weekends and holidays as needed.

Documentation Responsibilities: Collaborates with each caseload client and other available internal and external resources to develop/maintain treatment plans, transition plans, progress notes and appropriate updates in support of the health and recovery needs of the client. Properly documents all individual and group counseling sessions and completes the discharge paperwork/process and required agency assessments in timely manner. Also, maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.

And, other duties as assigned.

         

Education and Knowledge, Skills and Abilities

    

  • Drug and Alcohol Registration recognized by DHCS.
  • High School diploma or equivalent.
  • First Aid Certified within 30 days of employment.
  • CPR Certified within 30 days of employment.
  • A valid California driver's license.
  • Ability to obtain and maintain background clearance with successful discharge from probation or parole, and absence of drug/alcohol related arrests or convictions, for at least 3 years.
  • Culturally competent and able to work with a diverse population.
  • Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications.
  • Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency.
  • Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data.
  • Professionalism, punctuality, flexibility and reliability are imperative.
  • Excellent verbal, written, and interpersonal skills.
  • Integrity to handle sensitive information in a confidential manner.
  • Action oriented. Strong problem-solving skills.
  • Excellent organization skills and ability to multitask and juggle multiple priorities. Outstanding ability to follow-through with tasks.
  • Ability to work cooperatively and effectively as part of interdisciplinary team and independently assume responsibility.
  • Strong initiative and enthusiasm and willingness to pitch in whenever needed.
  • Able to communicate well at all levels of the organization including working with organization leadership and high-level representatives of partner organizations.
  • Able to work within a frequently changing project scope while maintaining overall direction and structured priorities.
