The Resource Center Manager coordinates the delivery of Enhanced Case Management including Medical, employment services, housing services, linkage to medical case management, and community referrals provided by the Resource Center. The primary duty of the Resource Center Manager is to ensure that high quality services are being provided to all patients. The Resource Center Manager ensures that patients receive timely, comprehensive, integrated medical, employment, and housing services, etc, and referrals to appropriate agencies.
Key Responsibilities
Supervision/Component Management:
- Recruits, interviews, hires, trains, and provides weekly supervision to Resource Center staff.
- Manages and assesses referrals from all clinic patients including health plan members to identify area of residence, acuity level, and functional limitations, and assigning cases considering these factors.
- Utilize electronic health record software to assign and monitor Case Managers/Care Coordinators caseloads.
- In collaboration with the Director of Operations prioritize integration of Resource Center clients with Medical and Dental services to ensure access to care.
- Review and approving documentation submitted by Case Managers/Care Coordinators for accuracy, completeness, and clinical quality.
- Support Case Managers/Care Coordinators in determining and locating appropriate medical and community resource linkages for members.
- Supporting Case Managers/Care Coordinators by providing non-clinical crisis intervention services in the field as needed.
- Providing direct case management services to health care plan members as needed.
- Develop and maintain effective working relationships with health care plan staff.
- Develop and maintain effective working relationships with Community partners.
- Collaborate with leadership team to identify programmatic issues and develop and implement solutions.
- Create agenda and lead regular staff meetings.
- Review and approve Case Managers/Care Coordinators timesheets and PTO requests.
- Manage and supervise volunteers as appropriate.
- Participates as needed in case conferences and other team meetings.
- Work with Director of Operations to manage hours of operations and productivity standards for all Case Managers/Care Coordinators.
Client Services/Case Management: Carries a small caseload of clients. Ensures that quality services are provided to all clients.
And, other duties as assigned.
Education and Knowledge, Skills and Abilities
Education and Knowledge, Skills and Abilities Qualifications:
- Bachelor’s Degree in Social Sciences, Liberal Arts or related field.
- First Aid Certified within 30 days of employment.
- CPR Certified within 30 days of employment.
- Excellent leadership skills, strong initiative and self motivated.
- Ability to teach/train others in all aspects of health care management, employment and housing counseling.
- Demonstrated ability in vocational/employment and housing counseling.
- 1 year or more experience providing case management for adults with multiple barriers to long-term economic self-sufficiency.
- Strong understanding of substance abuse, the therapeutic community treatment model and issues facing clients in recovery.
- Thorough knowledge of community resources.
- Ability to prioritize, utilize effective decision-making and problem solving skills.
- Strong organizational skills and detail oriented with ability to maintain databases.
- Exceptional written, verbal and interpersonal communication skills.
- Proficient in Microsoft Word, Excel, PowerPoint and Internet applications.
