Essential Functions:
- Contacts and tracks patient progress for the purpose of achieving established outreach program goals. This may include contacting patients via telephone calls, email and/or direct mailings to stress the clinical importance of receiving their recommended lab work and attending their medical appointments.
- At the request of the physician, may place lab order, coordinate preventive screenings, and/or schedule appointments in the electronic health record.
- Generate reports on site specific statistics to track findings as well as to compile data of outreach calls and outcomes to patients.
- Reviews patient charts and extracts pertinent clinic information for data survey collection tools.
- Transfers clinical information from patient charts into appropriate data survey collection tool.
- Performs chart audits, per training received from the Clinical Integration Department.
- Monitors compliance with policies and procedures relevant to clinical data management. Offers suggestions for improvements, as appropriate.
- Assists the RN Care Coordinator as needed with transitions in care, from both acute care and ED.
- Validates data for various disease registries.
- Validates data for the outcomes management program.
- Prepares utilization analyses.
- Adheres to established company policies and procedures, including the Corporate Compliance Plan, and follows state and federal regulations, such as OSHA and HIPAA guidelines.
- Performs duties in a professional manner wile exhibiting a courteous and cooperative demeanor to co-workers, management, and the public.
- Maintains strict confidentiality, both internally and externally.
- Performs other appropriate duties, as assigned.
Skills/Abilities:
- Familiarity of typical ancillary tests, physical exam findings, screening tests performed in primary care clinics.
- Knowledge of clinical care, nursing, and / or patients charts.
- Knowledge of computer systems and applications.
- Knowledge of health care administration principles.
- Skill in gathering, analyzing and interpreting information.
- Skill in written and verbal communication.
- Ability to exercise initiative, problem solving and decision-making.
- Ability to apply policies and principles to solve everyday problems and deal with a variety of situations.
- Ability to work effectively with physicians, patients, staff and the public.
- Must be well organized and detail organized and be able to work independently without much supervision.
- Must have the ability to work cooperatively as a member of office staff and team.

Qualifications Education:
CCMA, LPN, BS, Nursing
License & Requirements:
Active LPN license
Medical Assistant Certification
BS in health related field
And / or 3 - 5 years of medical office experience
Licenses & Certifications
Required- License Practical Nurse
