The Non-Medical Case Manager is vital to the Care Management team. This position provides supportive services to individuals living with HIV/AIDS to help them access essential resources, improve health outcomes, and maintain a high quality of life. The Non-Medical Case Manager assesses client needs and coordinates client services, ensuring clients have access to resources that support their stability and wellness. These activities are accomplished while adhering to all Virginia Department of Health Ryan White Part B, the Health Resources and Services Administration HIV/AIDS Bureau (HRSA HAB) and the Department of Housing and Community Development (DHCD) Housing Opportunities for People with HIV/AIDS (HOPWA) and Health Insurance and Portability and Accountability Act (HIPAA) requirements for client data compliance. The Non-Medical Case Manager/Data Coordinator will support the mission, vision, and values of the Healthy Community Health Centers (HCHC). Contributes to and participates in team Care Management culture.
1. Contributes to and participates in team Care Management culture.2. Conducts intake and ongoing assessments to identify clients’ non-medical needs, not limited to housing, transportation, food security, and social support.3. Completes appropriate intake forms for clients and determination of eligibility for Ryan White Part B services.4. Completes non-medical case management individualized service plans in collaboration with clients.5. Coordinates appropriate referrals with community resources, ensuring Ryan White and HOPWA are payors of last resort.6. Maintains client charts meeting all grant requirements.7. Coordinates with Medical Case Managers and healthcare providers to ensure holistic client care.8. Provides clients with education on available services and benefits, including Ryan White Part B and HOPWA programs.9. Participates in community outreach activities to identify potential clients and raise awareness of available services.10. Participates in Ryan White and HOPWA site visits, as determined by the Care Management Director and Assistant Director11. Participates in quality control processes to ensure criteria are always met.12. Maintains Care Management operations by following policies and procedures and reporting needed changes.13. Enters client-level data in varying databases, ensuring a high degree of accuracy and completeness.14. Observes the dignity, confidentiality and privacy of clients and maintains HIPAA guidelines.15. Participates in all required Ryan White and HOPWA trainings.16. Collaborates with partner organizations including insurance companies, community resources, consistent with HIPAA guidelines to communicate patient care needs and to maintain continuity of care as needed or required.17. Attends motivational interviewing, trauma-informed care and harm reduction trainings and will implement practices of each.18. Assists in the orientation of new personnel.19. Performs other duties and responsibilities periodically, which the Care Management Director or Assistance Director may deem necessary and appropriate.20. Must possess the ability to follow supervisor’s instructions and maintain regular attendance.21. Verbal and behavioral performance supports “managing up” of all team members.Requirements
• High School Diploma or equivalent degree is required AND one year of experience working with people living with HIV and/or health care training (e.g. certified medical assistant, medical clerk).
• Exceptional knowledge of Microsoft Office® applications.
• Experience maintaining a professional level of privacy in accordance with HIPAA and other relevant regulations.
• Efficient work management including planning, organizing, and prioritizing.
• Strong written and oral communication skills.
• Experience working with electronic databases.
• High degree of interest in continuing educational events.
• Ability to enter and maintain information with great attention to detail and accuracy.
• Great team player with strong interpersonal skills.
• English/Spanish bilingual skills preferred.
