What are the responsibilities and job description for the Case Manager position at Healthy?
Our Case Manager identifies, engages, and transports individuals in need to our clinics so they can receive HIV awareness and prevention education and ensure they are provided with STD and HIV testing, counseling, and referrals. This program provides risk reduction awareness and advocacy and a different approach to life through street outreach, intervention counseling, and HIV prevention education.Essential Duties and Responsibilities : Engage in street and community outreach.Recruit participants for HIV / STD / Hep C testing, treatments, and counseling services.Engage street contacts in risk reduction discussions.Distribute HIV / HEP C / STD risk reduction information.Make home visits and reach out to persons / patients that present an adherence problem to treatment.Ensure data collection in a timely, accurate, and consistent manner.Distribute preventive materials including safe sex kits.Provide outreach and recruitment activities including but not limited to representing the program and agency at community and committee meetings, providing educational workshops, and social marketing activities.Deliver patient medication as needed.Perform other duties and responsibilities as assigned.Computer Equipment & Software Skill Requirements : Full knowledge of Windows Office Suite, particularly Microsoft Word and Excel.Basic understanding of computers.Requirements : A high school diploma or GED.Workplace Civility Requirements & Attributes : Represent the company with a positive demeanor both internally and externally.Participate fully as a team member, completing all requirements assigned in a timely manner.Treat all employees, clients, and vendors with dignity and respect.Work cohesively with team members and all co-workers.Display a positive attitude through supportive behaviors, problem-solving, and communication skills (both verbal and written).Act in a professional manner at all times to maintain a positive work environment.Utilize excellent written and oral communication skills.Maintain a professional client relationship in order to properly represent IMG Health.Reasonable accommodation may be offered to enable qualified individuals with a disability to perform the essential functions of the job.#J-18808-Ljbffr
