What are the responsibilities and job description for the Connected Care Coordinator - HYBRID position at Heart and Vascular Care?
We are looking for a Connected Care Coordinator to join our team!

Monday-Friday, 8AM - 5PM, no nights or weekends
This is a Hybrid Role - 4 days from home / 1 day in our Dawsonville office - after 2 to 4 weeks of hands-on training.
- Must have prior experience working with remote patient monitoring and/or principal care management, and triage.
- You will be working over the phone and via a telemetry portal, being comfortable with technology and helping patients with the equipment is a plus.
- Relationship building is an important part of this role with patients.
- Previous work with Connected Care is preferred.
- Previous MA experience is desirable.
Connected Care Coordinator duties may include but not limited to:
- Communicate both with providers, staff, patients, insurance companies, reference labs and other medical practices
- Answer inbound calls, give direction, and provide medical advice
- Preforming telephone outreach and be the main contact person to identified population in effort to improve the quality of life
- Utilize a database to track patients in the Chronic care program with hypertension, Coronary arteriosclerosis, A-Fib, congestive heart disease and Hyperlipidemia
- Making a care plan to keep patients on track with appointments and medication compliance
- Perform monthly outreach calls to stay connected with the patient and their health
- Communicate with the Providers regarding any concerns the patient may have
- Assist patient with medication concerns, insurance concerns
- Communicate with the primary care physician with any updates or concerns
