Case Manager | Full-time
Heart House Inc. Case Manager will support the residents and operations of Heart House Inc. in accordance with policies and procedures.
· Follows sound ethics and morals that guide all professional service/conduct decision-making processes. Maintain confidentiality.
· Maintains consistent hours to help Heart House Inc. business and be flexible according to the organization's potential needs. Availability to respond to off-hours requests as needed.
· Effectively and professionally communicate via phone, electronic mail, phone, or in person, with staff, community members, and clients.
· Perform client interviews and assessments, and support ongoing casework including goal planning and resource navigation.
· Develop and maintain physical and digital copies of client documentation, records, and care plans (Working knowledge of HMIS, Eccovia Clientrack, and IHCDA).
· Assist in client referrals and resource navigation with community partners and client services.
· Demonstrates effectiveness in working with individuals of all backgrounds and ages. Prioritizes cultural sensitivity and equality when managing caseload.
· Assist clients in job referrals and placement.
· Assure clients are attending required programs and adhering to Heart House rules and policy.
· Assistance and guidance in financial planning and client budgeting.
· Maintain agency statistics and data.
· Familiarity with local, state, and federal programs regarding insurance, food stamps, and social services.
· Familiarity with Trauma Informed Care models, peer coaching, substance abuse, mental health barriers and motivational interviewing. Lived experience is a plus.
· Other duties and projects, as assigned by the Executive Director.
Minimum Qualifications (Knowledge, Skills, and Abilities)
· High School Diploma or equivalency.
· Working knowledge of community resources and housing insecurity/homelessness issues.
· Approaches daily responsibilities with a positive attitude.
· Sitting, standing, and walking for brief periods.
· Good written and verbal communication skills.
· Ability to work independently.
· Organizational and time management skills.
· Ability to build and maintain relationships with community partners.
· Demonstrates skill and sensitivity to cultural differences and treat clients with respect.
· Ability and willingness to work in a community setting(s).
· Ability to work as part of a team and communicate with a supervisor(s).
Job Type: Full-time
Pay: $18.00 - $20.00 per hour
Expected hours: 40 per week
