Summary : The Nutrition Program Manager supervises registered dietitians and leads nutrition-related initiatives to better patient health outcomes. This role is an exempt position responsible for providing individual and group medical nutrition therapy to treat patients with various disease states within primary care clinics. The Nutrition Program Manager will interact with the interdisciplinary team members and serve as a nutrition resource to the medical team at each practice site.

Job Duties , these are considered essential to the successful performance of this position :
- Assesses patient’s medical history, including nutritional status, diet history, and food habits. Conducts individual or group sessions to discuss patient’s nutritional needs and determine risk factors. Confers with physician and other clinicians as appropriate on effect of nutritional status.
- Develops evidence-based and culturally-appropriate nutritional plans for patients. Presents information to patient / family including recommendations for special diets and diet techniques to ensure proper preparation and nutritional intake.
- Documents assessments in patients’ electronic health record and bills insurance for services provided.
- Contacts insurance companies to determine coverage for dietary services such as Medical Nutrition Therapy and Intensive Behavioral Therapy for Obesity as needed.
- Serves as a preceptor or assigns staff to act as preceptors for dietetic interns.
- Supervises dietitians for time and attendance, performance evaluations, development, training, and interviewing.
- Completes chart reviews, observe visits, and complete other tasks to assess and improve competency of other staff in your department.
- Primary champion for the Mid-Ohio Farmacy program and acts as liaison with food access and subsidy programs.
- Develops wellness text reminders targeted toward patients with diabetes or hypertension enrolled in our education program.
- May be asked to lead interdisciplinary meetings such as patient panel meetings and other interdisciplinary groups to improve diabetes and hypertension outcomes.
- Develops and presents nutrition-related presentations to providers and community organizations as needed.
- Works with other department managers to ensure strong collaboration among ancillary care providers and improve visit workflows.
- Participates in quality improvement initiatives as needed.
- May manage other staff related to chronic disease or nutrition programs.

Job Qualifications (Experience, Knowledge, Skills and Abilities)
Education : Bachelor's degree in nutrition, food service, or community health. Master’s degree preferred.
Experience : Three years of experience as dietitian. Experience in outpatient setting preferred. Management experience preferred.
Registration / Licensure : Registered dietitian as certified by the American Dietetic Association’s Commission on Dietetic Registration. Licensed dietitian in Ohio

Heart of Ohio Family Health operates in multiple locations, in Columbus, OH. All facilities have a medical office environment with front-desk reception area, separate patient examination rooms, nursing stations, pharmacy stock room, business offices, hallways and private toilet facilities. All facilities are ADA compliant.
This position’s primary work area is the clinical portion of the facility; ie, patient examination rooms, hallways, and other clinically assigned areas.
