Heart of Texas Hospice is looking to hire a full-time Registered Nurse Case Manager to join our amazing team. The RNCM will assist with hospice patient visits in the Killeen/Temple and surrounding areas. The RNCM will work with a team of healthcare providers to ensure our hospice patients are receiving quality care.
Responsibilities:
- Conducting routine visits, admissions visits and/or follow-up visits at home or other facilities settings.
- Coordinating, assessing, and implementing patients' care.
- Providing accurate and timely documentation of patient services to reflect plans of care using an extremely user friendly EMR.
- Monday thru Friday work week with occasional On Call 2-3 times per month Mon-Thur only.
- Flexible schedules.
- No weekends.
