What are the responsibilities and job description for the Program Manager/Our Community Our Future (OCOF) System of Care Coord. position at Heart of Texas Region MHMR Center?
OCOF System of Care Coordinator will coordinate and facilitate communication and community engagement, communication amongst OCOF partners and other community entities, and community development in line with OCOF and HOTBHN’s mission in an effort to optimize continuity of care for children, youth, and young adults throughout the six-county region.
EXAMPLES OF WORK PERFORMED
- OCOF System of Care Coordinator will coordinate and facilitate scheduling for all OCOF meetings to include steering committees, large groups, and community partners.
- OCOF System of Care Coordinator will attend community engagement meetings and events to collaborate across systems.
- OCOF System of Care Coordinator will facilitates cross-spectrum support across the Heart of Texas Region to reduce service duplication, foster collaboration, identify blended funding strategies, and coordinate at the macro-level optimal care for at-risk children and families, and serve as connectors to lived experience support.
- Other duties as assigned.
