What are the responsibilities and job description for the Community Support Manager position at Heart to Heart Home Care?
Based out of South Jersey office
May require travel throughout state for purposes of Program Development
Will require travel to consumer locations throughout Heart to Heart Home Care service area
May require evening hours
Will work in the field at supervisors’ allowance
Requirements:
Must be 21 years of age or older
Three or more years of experience working with people with developmental disabilities
Case Manager must be cleared to work with no disqualifying factors in accordance with State and Federal guidelines
Must have a vehicle in good working order to travel to needed locations
NJ Driver’s License in good standing
Skills:
Excellent written and verbal communication skills
Knowledge of sound program practices and program development
Good computer skills
Strong organization
General Statement of Duties:
The Case Manager will oversee the development and implementation of non-licensed programs at Heart to Heart Home Care. This will result in programs and services that accomplish the mission and goals as well as meet consumer needs. The Case Manager will oversee accountability systems such as Continuous Quality Improvement, Management Information System and the Safety Committee, ensuring routine consumer feedback is obtained and required internal and external reports are prepared and submitted.

Pay: $45,000.00 - $55,000.00 per year
