Direct Support Professional – In-Home (Floater)
Heart to Heart Home Care is seeking professional, compassionate Direct Support Professionals who are enthusiastic about making a positive difference in the lives of our consumers in their homes.
Position Overview
This is primarily an in-home care position. As an In-Home Floater, you will be temporarily assigned to our group homes or day programs only while waiting for an in-home case to become available. Once an in-home case opens, you will be immediately transitioned to provide individualized support to consumers in their homes.
Responsibilities and Duties
- Provide direct support to consumers primarily in their homes
- Provide support, transportation and assistance for each individual to enable him/her to complete daily routines in a safe, healthy, productive and dignified manner
- Assist with toileting, bathing, dressing, laundry, supervision and recreation
- Encourage independence by providing both the opportunity and the appropriate levels of skill training for individuals in categories such as self-care, household chores, social skills and recreation skills
- Complete documentation accurately and promptly
- Maintain professionalism and quality care standards
Critical Requirements
- PRIMARILY IN-HOME POSITION - This role is focused on in-home care, with temporary assignments to group homes or day programs only while awaiting an in-home case
- OPEN AVAILABILITY IS ESSENTIAL - Must be available to work various shifts including days, evenings, weekends, and holidays
- FLEXIBILITY IS MANDATORY - Transitions from temporary group home/day program assignments to in-home care may occur spontaneously and without advance notice
- Must be prepared to switch to an in-home case immediately when one becomes available
- Reliable transportation and proof of valid car insurance to travel to client homes and temporary assignments and to provide transportation to client appointments/activities
Job Type: Full-time
Pay: $18.25 - $22.00 per hour
Expected hours: 40 per week
