Position Overview:
Heart to Heart Health and Wellness, a mental and behavioral health agency is looking to hire a community based Case Manager(s), also commonly referred to as a Qualified Mental Health Specialist (QMHS) for the Cleveland, Ohio area. Our Case Managers provide individualized case management services to an assigned caseload of clients, while also providing ongoing monitoring, support and behavioral health support services to children and youth with emotional trauma, as well as to serve their families. Case Managers will engage the children and family in scheduled (routine and intensive) services, often times in a designated school or other establish location based on client needs.
As a Case Manager, you will apply interventions that focus on the individual’s ability to succeed in the community; to identify and access needed services; and to assist the individual with making improvements in their respective schools, work places, and community. This position requires collaboration with community resources, school administrators and programs to best serve our clients need.
Role Responsibilities:
- Contributes to Heart to Heart's mission, vision, and program philosophies by way of work product and professional behavior with both internal and external consumers
- Adheres to all current federal & state laws, as well as Heart to Heart’s Policies and Procedures, including the safeguarding of confidential healthcare information and compliance with the Health Insurance Portability & Accountability Act (HIPAA)
- Demonstrates an awareness, appreciation and respect for diverse cultures and individual differences of clients and fellow employees
- Provide case management, mental health services and life skills coaching as appropriate to assigned caseload
- Meets face-to-face with clients in their home, school, office and community for the provision of all services and keeps the family and team members informed on progress toward achieving Service/Treatment Plan goals
- Enhance access to mental health services by facilitating the referral process to meet goals as outlined in the individual treatment plan
- Effectively engage clients and their families through treatment planning and progress reviews
- Provide coordination and assist in crisis management and client stabilization as needed
- Link clients to community resources and/or other school and family support programs
- Accurately complete service documentation in accordance with agency, funder, and state standards
- Attend supervision, staff meetings, and training as required
- Sustain a weekly billing productivity of a minimum of thirty hours per week
- Maintains all client documentation in the electronic record system and ensures adherence to requirements set forth in all state and federal rules and regulations
- Other duties as assigned
Position Qualifications:
- Minimum High School Diploma; Bachelor’s Degree preferred
- Valid driver’s license and good driving record, auto insurance, reliable transportation
- Excellent oral and written communication skills
- Proficiency in the use of Microsoft Office Suite
- Ability to work a flexible schedule in accordance with needs of populations served
- Knowledge of available community resources
- Positive attitude and a TEAM player
- Ability to support the agency's culture, growth, and success through communication, accountability, and positivity
- Ability to appropriately accept feedback through the supervision process - thus displaying the willingness to learn, grow, and improve
- Effective conflict resolution skills
Previous Experience:
- Mental Health services: 2 years (Preferred)
