Heartis Village of Peoria is currently seeking a compassionate LPN for our Generations Program Director. The ideal candidate will be responsible for the overall operation, program development, and quality assurance of the Memory Care program, ensuring that residents with memory impairments receive the highest standard of care. This role requires a strong understanding of dementia care practices, excellent leadership skills, and a commitment to enhancing the quality of life for our residents and their families.

Responsibilities include

- Oversee the daily operations of the Memory Care Unit, including staff management, program implementation, and resident care.
- Recruit, train, and supervise memory care staff, providing ongoing professional development and performance evaluations.
- Foster a positive and supportive environment for both staff and residents.

- Design and implement individualized care plans for residents based on their unique needs and preferences.
- Develop and manage engaging activities and programs that promote cognitive function, social interaction, and emotional well-being.
- Ensure compliance with all state and federal regulations pertaining to memory care services.

- Build strong relationships with residents and their families, providing education and support regarding memory impairments and care strategies.
- Conduct regular assessments of residents’ needs and adapt care plans as necessary.
- Act as a liaison between residents, families, and the healthcare team, advocating for the best interests of each resident.

- Monitor the quality of care provided to residents, ensuring adherence to best practices and quality standards.
- Conduct regular audits and assessments to identify areas for improvement and implement corrective actions as needed.
- Maintain accurate and up-to-date documentation, including care plans, incident reports, and staff training records.

Requirements include:

-Nursing license in the state of Illinois
- Minimum of 3-5 years of experience in memory care or geriatric services, with at least 2 years in a management or leadership role.
- Strong knowledge of dementia and Alzheimer’s disease, including best practices in care and therapeutic interventions.
- Excellent communication and interpersonal skills, with the ability to collaborate effectively with staff, residents, and families.
- Compassionate and patient demeanor, with a passion for enhancing the quality of life for individuals with memory impairments.
