- Job Summary:
This is a professional position responsible for family support assistance with caregivers (relatives, non-relatives and kinships).
- Essential Functions:
- Establish and maintain positive working relationship with HFC internal and external customers
- Work collaboratively with the kinship and caregiver supportteam
- Assist with benefits, resources identification/development and service navigation
- Provide support, resources and referral information for placement support and stability
- Facilitate ongoing continuing educational trainings to further build parenting skills
- Participate in meetings and/or staffing’s involving or regarding caregivers as appropriate with goal setting, barriers, placement stabilization and permanency planning
- Assist with reaching out to case management and provider staff to ensure service engagement is occurring
- Assist with school registration, school transportation and child care registration as needed
- Work with other departments collaboratively that will also be supporting caregivers regarding benefits and permanency goals
- Lead and/or assist with support groups as needed
- Ensure quality and timely documentation of work
- Perform special projects as assigned
- Qualifications Required:
Bachelors degree in Social Work or related field.
- Qualifications Preferred:
Two years of experience working with children, adolescents and families in a child welfare setting is preferred.
