Job Summary
The Palliative Care Nurse Practitioner will be responsible for facilitating palliative care for Gentiva Palliative Care patients, ensuring comprehensive, individualized treatment plans. This role requires excellent clinical skills, ethical judgment, and collaboration with the Palliative Care Medical Director and other team members.
Key Responsibilities
- Provide diagnosis, treatment, consultation, and follow-up under the direction of the Palliative Medical Director (if required)
- Work collaboratively with the Palliative Care Medical Director and other palliative team members to deliver palliative care
- Take responsibility for patient palliative care from initial assessment through discharge
Requirements
- Master's degree in Nursing from an accredited school of nursing or equivalent
- Nurse Practitioner specialty in Family or Adult Medicine preferred
- Practice experience with Part B billing
- Collaborative agreement in place with a supervising physician or willingness to enter one
