Travel Percentage:    Up to 75%

Description

  1. Training the individual to follow directions, attend to task, multi-task, complete tasks, problem solve, work safely, increase productivity, adapt to work routines, and carry out assigned duties in an effective manner.

  2. Helping the individual to acquire appropriate attitudes and work habits, including instruction in socially appropriate behaviors on and off the job site.

  3. Assisting the individual to adjust to the demands of the workplace.

  4. Familiarizing the individual with job production and performance requirements.

  5. Providing travel training, including the use of public and para-transit systems.

  6. Transportation between activities.

  7. Instruction in the appropriate use of job-related facilities (e.g., lounge areas, lunch rooms/cafeterias, and rest rooms).

  8. Assessing the individual to determine his or her work interests and skills.

  9. Instruction in the use of technology that can assist in developing job skills and meeting workplace expectations.

  10. Assisting the individual to experience a variety of employment options within the community.

  11. Implementing individuals' plans, consistent with their specific services, goals and outcomes. Participating in the planning and implementation of activities, both at the program and in the community, that address these goals and outcomes.

  12. Promoting choice and self-determination to support individuals to direct the course of their own lives. Ensuring that services and goals reflect what the individuals want for themselves.

  13. Respecting and affirming the human rights, civil rights and responsibilities of the people supported.

  14. Supporting and teaching individuals according to their services, goals and outcomes. Actively engaging individuals in skills development, socialization and task participation across all settings. Ensuring that supports are provided with consideration for each person's individualized needs and routines.

  15. Promoting and supporting the inclusion of individuals in their community according to their unique needs, interests, and desires. Modeling and encouraging interaction between individuals and community members. Fostering individuals' personal growth by assisting in forming and maintaining friendships with neighbors, co-workers and peers.

  16. Assisting in maintaining positive communications with family members, advocates, and service providers as per program needs.

  17. Transporting individuals or assisting them in taking public transportation, as required.

  18. Participating in planning recreational activities and outings.

  19. Participating as requested in the Program Planning Team (PPT) meetings.

  20. Documenting necessary information and data in areas such as medical, behavioral, individualized services, goals and outcomes, fire safety, community inclusion, communication books, etc., and ensuring their accuracy and completeness.

  21. Following all agency policies and procedures, as well as state and federal regulations, to maintain compliance. Completing all written reports and documentation within required timeframes.

  22. Providing appropriate levels of supervision on an individualized basis

  23. Actively engaging in all program services such as behavioral interventions, community inclusion, emergencies, etc. as needed and required.

  24. Safeguarding and respecting the confidentiality and privacy of all individuals.

  25. Reporting all incidents to program management immediately upon discovery as per HeartShare policy, and OPWDD & Justice Center requirements.

  26. Ensuring that equipment and supplies are appropriately and safely stored.

  27. Maintaining professionalism by using positive approaches when handling difficult situations. Avoiding gossip and negative discussions. Being flexible and adapting to change.

  28. Advocating appropriately and professionally on behalf of individuals.

  29. Serving as a role model for individuals and co-workers. Abiding by division's Personal Appearance Guidelines. Conducting oneself in a professional manner at all times.

  30. Remaining sensitive, respectful and responsive to cultural differences of all individuals, program staff, and the surrounding community.

  31. Participating in staff meetings, in-service trainings and on-site workshops as required for professional growth and development. Attending external trainings and workshops at direction of direct supervisor or program management. Maintaining certification, CPR/First Aid and SCIP, as required. 

  32. Representing HeartShare both within and outside the organization in a manner that promotes the mission, vision and values of the agency. Personally adhering to HeartShare's Standards & Values Statement, in addition to all other agency policies and procedures.

  33. Undertaking any additional tasks and initiatives as per the specific needs of the programs and individuals and as assigned by supervisor.

  34. Meeting or exceeding the DSP Core Competencies Standards by the end of the 1st year of employment.

Qualifications


  • Minimum of high school diploma or GED. Some college experience preferred.

  • One or more years' experience working with individuals with intellectual and developmental disabilities preferred.

  • Ability to travel throughout the five boroughs of NYC
