Job Summary
The Speech-Language Pathologist evaluates, plan goals and treats individuals with speech, swallowing and communication disorders. The Speech Therapist is an integral part of the Interdisciplinary Team providing an improved quality of life for the affected individuals.
Job Responsibilities
- Evaluates the degrees of function and disability of the individuals.
- Provides appropriate professional treatment in order to improve and/or sustain the individuals functioning level and ability to communicate.
- Provides written evaluations to be placed in the individuals medical record, including the following information:
- Date and signature
- Individual's medical history
- Individual's referring diagnosis
- Individual's secondary diagnosis
- Present level of function of receptive expressive language skills and motor control
- Prognosis and recommendations
- Ensures that the individual's physician and members of the Interdisciplinary Team are informed of the goals and expectations for the consumer's progress.
- Provides written and electronically signed documentation and of the individual's response to treatment via a speech-language pathology progress note in the individual's EHR.
- Performs hearing screenings on all new individual's, as requested.
- Remains sensitive and ensure staff responsiveness to cultural differences in Heartshare Wellness service population.
- Undertake any task and initiative as assigned by the Medical Director and Clinic Director
Qualifications
Qualifications
- Master's Degree from an accredited college or university from the Speech and Language Pathology program. Approved by the American Speech Hearing Association.
- Holds Certificate of Clinical Competence (C.C.C) as a result of passing the national certification examination in speech pathology and having worked for paid professional services in the field of speech and language pathology for a minimum of one year.
- Holds a current license issued by New York State Department of Education to practice as a Speech and Language Pathologist and is registered with the State of New York.
- Minimum of one year experience as a certified licensed speech pathologist.
