LPN Home Health Nurse:
Overview
Responsible for providing skilled nursing services to clients in their homes under the direction and supervision of a registered nurse. Must assess, plan, implement and evaluate clients’ needs and care. Performs duties and responsibilities in a manner consistent with our mission, values, and Standards.
LPN Home Health Nurse Top Duties and Qualifications:
A Home Health LPN is responsible for traveling to a patient’s home to administer their services and helping clients maintain their independence.

Home Health Nurse duties and responsibilities:
Home Health Nurses provide a variety of tasks during patient visits depending on the specific plan of care and type of patient. They often have the following duties and responsibilities:
· Assess the state of their health and chart observations of the patient’s condition at each visit.
· Reviewing Physician-prescribed medication,
· Set up medications in weekly medication container
· Taking and recording vital signs
· Provide education to clients and families on how to carry out routine care procedures like administering medication, checking blood sugar levels and changing bandages
· Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
· Keep the patient comfortable through palliative care.
· Recommend tools or devices that may improve the quality of life for the patient.
