Job Description
The Helen Ross McNabb Center is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will be responsible for providing case coordination services to clients using an integrative approach.
This role involves interfacing with agencies to increase cooperation and consistency between home, school, work, community, and behavioral and physical health providers. You will serve as the primary care coordinator that provides case coordination services to a predetermined number of children and/or adults.
Your duties will include functions as a member of a treatment team to plan, implement, and evaluate successful interventions for children, adults, and/or families. Key Responsibilities:
- Provide case coordination services using an integrative approach
- Interface with agencies to increase cooperation and consistency between home, school, work, community, and behavioral and physical health providers

Qualifications
To be successful in this role, you will need to have a Bachelor's degree in a health-related field such as counseling, psychology or social work. You must also have course work and/or experience in cultural diversity, human development, etiology and treatment of medical disorders and mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and intellectual disability.
In addition, you will need to have a valid driver's license with F-Endorsement and maintain a dependable vehicle for conducting Center business.
Preferred qualifications include computer experience and certification in verbal and physical de-escalation techniques.
