Ending homelessness on youth, one family, one individual at a time.Benefits of working at HELP of Southern Nevada15 paid HolidaysBirthday HolidayPaid sick and vacation time403B90% of the employee only premium is paid for Medical, Dental, Vision, and Life InsuranceEmployee Assistance Program (EAP)Family Housing Services (FHS)Family Stabilization Case ManagerHours : Monday - ThursdayStatus : Full-TimeReports to : FHS Supervisor / ManagerAbout the Department : The Family Housing Services department is made up of housing for families with children, pregnant women, and youth who are at-risk of becoming unhoused or unhoused living in shelters, vehicles or on the streets.Job Summary : The Family Stabilization Case Manager reporting to FHS Supervisor will be responsible for providing direct case management for Anthem members for services in unhoused prevention, applications for rental and / or utility assistance, and Rapid Re-Housing (RRH) due to financial hardship.Conduct initial interview / intake and client needs assessments based on referrals receivedDetermine eligibility for homeless prevention, rent and utility assistance, and RRH based on criteria set in grantCollect appropriate documents and information from client(s) to assess and document eligibility (i.e. Identification of household members, income verification, crisis / need, lease, sustainability, MCO designation, etc.)Provide case management and supportive services to clients participating in housing programCreate individualized case plans with each adult member of the household, based on clients' needs, with the goal of achieving self-sufficiencyReview / update client's case plan monthly, and track outcomesConnect client with wrap-around services through other programs offered at HELP, including WORK, Diaper Bank, holiday programs, etc.Connect client with mainstream programs, including but not limited to, Medicare, WIC, TANF, SNAP SSI / SSDIProvide client with referrals and resources for other social services and partnering agenciesEnter case notes in daily into HMIS, reflecting activities of the client's progressCreate and maintain accurate client filesMaintain clear boundaries with all clients and providersAlways be vigilant with yours and the organizations ethical standardsAll other job-related duties as assigned by management for needs of the organizationThe ideal candidate will possess the following knowledge, skills, and abilities : Knowledgeable of information and community resources to connect clients with the intent to meet their individual needsKnowledge of local social service agencies and mainstream programsKnowledge of social service work, interviewing / referral techniques.Understand and follow complex instructions and procedures. Analyze situations accurately and adopt an effective course of action.Must communicate effectively with persons from a variety of social, cultural and economic backgrounds, and possess a demonstrated understanding and sensitivity for those groupsEffective written, verbal and listening skillsIndependently establish, maintain, initiate cooperative working relationships with all organization contactsAbility to maintain order and exercise appropriate judgment in crisis situationsAbility to be flexible and quickly adjust to shifting prioritiesOrganize work schedule and effectively manage timeAbility to understand and follow all agency policies and proceduresGeneral computer knowledge including; Microsoft office suiteAbility to work flexible hours, as dictated by consumer case loadAbility to perform accurate mathematical equations to calculate client's rent, utility costs, and to request funding to pay those billsRequired Qualifications : Bachelor's degree in social work or related field, OR equivalent combination of education and experience Experience working with unhoused individuals, families and childrenActive NV Drivers License and Clean Drivers ReportPreferred Qualifications : Bilingual English and SpanishPrevious experience working in a social services setting
