Helping Hand Nurse is a Home Health Care agency servicing the greater Philadelphia Area and surrounding counties. We are seeking a compassionate and detail-oriented community coordinator to perform pop-in visits for home care clients. This role ensures that caregivers provide high-quality, consistent care in alignment with our mission and standards.
The Coordinator will be responsible for:
- Conduct supervisory visits in clients' homes to observe and evaluate caregiver performance
- Assess client satisfaction and ensure care plans are being followed
- Provide feedback and support to caregivers to improve the quality of care
- Document visit findings and report any issues to the supervising nurse or management team
- Coordinating patient care with field staff, doctors, and organizations
- Conducting orientation/training for new caregivers
- Ensure compliance with caregiver clockin and clockout requirements
- Provide direct care to clients when necessary, including assistance with activities of daily living (ADLs).
- Ensure compliance with all company policies, procedures, and state regulations.
- Developing relationships with different Service Coordinators
- Increasing satisfaction and caregiver/patient retention
- Creating patient schedules and assigning staff to appropriate schedules promptly

Qualifications include:
- Current Certified Nursing Assistant (CNA) license in Pennsylvania.
- At least 1 year of experience in home healthcare or a related field.
- Strong observational and communication skills.
- Ability to work independently and manage time effectively.
- Reliable transportation and a valid driver's license.
- CPR certification (preferred).
- Demonstrate effective interpersonal skills as well as sound judgment and good decision-making skills
- Bilingual (Spanish) preferred but not required
