Job Overview
We are seeking a dedicated and detail-oriented Qualified Professional to join our team to oversee our clubhouse model Psychosocial Rehabilitation (PSR) program. Psychosocial Rehabilitation is a structured day program for adults with psychiatric disabilities designed to enhance the vocational, educational, leisure and self-help skills of the participants. This program focuses on developing skills and resources related to life in the community and increasing the participants’ ability to live independently, manage their illness and participate in community opportunities related to functional, social, educational, and vocational goals.
Purpose of the Position:
The PSR Qualified Professional (QP) will facilitate and oversee the day-to-day operation of the PSR program.
Qualifications/Competencies:
- Bachelor’s degree (Not Human Services field) & 4 years full-time experience with population served OR
- Bachelor’s degree (in Human Services field) & 2 years full-time experience with population served OR
- Master's Degree or Higher & 1 years full-time experience with the population served.
- Facilitating relationships and serving as a link between the company, parents, guardians, local agencies, and the community
- Minimizing the negative effects of psychiatric symptoms or substance dependence that interfere with the recipient's daily living and personal development, providing supportive counseling.
- Supporting the client in the development of various skill building activities, including daily and community living skills, socialization skills, adaptation skills, and behavior and anger management
- Participating in a first responder on-call system available to consumers and/or his/her natural support network on a 24/7/365 basis; coordinates "first response" resources according to consumer need and the PCP
- Performing Case Management functions of linking and arranging for services and referrals
- Working closely with other clinical/professional staff to maintain communication and providing feedback, standardizing procedures, and expediting PCP implementation.
- Ensuring that all initial reauthorizations for services occur in a timely fashion.
- Monitoring utilization of service to ensure that it is effective, appropriate, and within the limits set forth in both rule, PCP, and the service authorization.
- Must a have valid North Carolina Driver’s License.
- Provide psychosocial rehabilitative and pro-educational activities for high-risk intervention consumers. Engage assigned consumers in skill building including daily and community living skills; socialization skills, adaptation skills, development of leisure time interests/activities, symptom management skills; wellness education and work readiness; education substance and behavior and anger management techniques.
- Actively participate as a team member in Person Centered Planning and Development. Documents consumer’s daily progress note modeling, positive reinforcements, redirection, de-escalation, anticipation guidance, etc. or recreational activities when used as a strategy to meet clinical goals (e.g., card playing/bingo). Maintain accurate consumer information regarding goals, interventions, service plans and significant incidents and submit all documentation/progress notes to agency through electronic system.
- Participate in clinical and administrative meetings regarding service implementation and consumer progress and maintain communication with supervisory staff regarding consumer issues and service delivery. And complete all agency training.
- Advocate for consumer rights and needs and help consumers link and access community resources.
- Ensure compliance with the agency Policy and Procedures regarding consumer safety and consumer rights. Maintain incident and accident reporting procedures. Review and function in accordance with the policies and procedures of Helping Hands MH/DD Services.
- Provide supervision to AP, PP staff.
