The Substance Use Specialist, as a key member of the Assertive Community Treatment (ACT) team, is responsible for assessing, developing, and implementing substance use goals for individuals with severe mental health challenges. The specialist collaborates with team members, and community partners to support clients in substance use recovery.
Responsibilities
- The ACT Substance Abuse Specialist provides skills training, case management, crisis intervention to participants with co-occurring disorder
- Follow evidenced based practices of psychosocial rehabilitation.
- Conduct all phases of psychosocial rehabilitation and outreach (engagement, assessing
strengths, goal planning, safety planning, evaluating and summarizing progress)
- Providing and coordinating substance abuse assessment
- Providing and coordinating substance abuse treatment
- Creating and reviewing relapse prevention plans, WRAP and safety plans
- Assisting with creating natural supports and accessing community resources that support
recovery
- Teaching Illness, Management and Recovery with individuals with co-occurring disorders
- Providing and coordinating substance abuse services tailored to needs and stage of change
- Participate in integrated team meetings
- Provide back-up services for peer team members
- Actively support participant self-reliance
- Provide skills training for medication management, financial management, and for all
activities of daily living
- Provide case management
- Provide crisis intervention
- Collaborate with other staff members, community agencies/supports, and family members about participants mutually served
- Complete required documentation in a timely fashion including the strengths assessment;
personal treatment plan; safety plan; domain changes; and progress notes
Minimum Requirements
- Must be able to obtain credentials as QMHA: BA/BS in mental health, social services, or related field or at least three years relating to or assisting individuals with mental illness.
- At least one year of demonstrated experience with substance abuse population required;
- CADC preferred; must be able to obtain CADC within one year of hire.
