Quality Care Navigation Nurse (RN or LPN)
Provides and coordinates professional nursing care for Hematology Oncology Associates patients following established standards and practices.

Requirements:
EDUCATION: Graduate of an accredited school of nursing.
EXPERIENCE: Chronic Care Management or other Case Management Experience: 1 year (preferred)
CERTIFICATE/LICENSE: Possession of a New York State Registered Nurse or Practical Nurse license. Maintain current CPR certification.
