Job Overview:
The Referral Specialist- screener will be responsible for the overall referrals for the admissions process. We are looking for a meticulous and organized Referral Specialist to join our agency. As a Referral Specialist, you will be responsible for managing patient referrals, coordinating with the clinical team and directors, and ensuring seamless transitions of care for our patients. Your role requires considerable contact and collaboration with internal program directors/managers, clinical, and support staff. The incumbent will be crucial in facilitating efficient communication between the agencies to optimize the referral process.
Schedule: Monday to Friday - 9:30 am to 6:00 pm
Responsibilities, but are not limited to:
- Address incoming calls/emails regarding referrals from different agencies.
- Collaborate with referral sources. Maintain a harmonious relationship with external agencies.
- Obtaining necessary documents for clients.
- Scheduling phone screening appointments for potential clients.
- Uploading documents into emr system.
- Reviews/Tracking referrals records and ensures accuracy/completeness – coordinates with referring offices for clarification/corrections as needed.
- Support Program manager with scheduling, and administrative duties.
- Maintain files to meet all internal, state, and Federal quality and compliance standards.
- Follows up on all client referrals and correctly documents communication to ensure no gaps in client care.
- Other office responsibilities assigned by supervisor as deemed necessary.
- To ensure that individuals are both informed about and able to access the full range of medical and treatment opportunities or other support services, such as legal, financial, educational, vocation or employment counseling, which may be available to them at the time the individual chooses to use them.
- Keeps daily statistics on the client population.
- Ensures the confidentiality of all records, materials, and communications concerning the identity of clients.
- Transport clients as needed.
- Performs all other duties as directed by the Director.
Qualifications:
- Previous experience in a healthcare setting, preferably in referral coordination or a related role.
- Familiarity with confidentiality (CFR 42), HIPAA regulations and patient privacy laws.
- Ability to work collaboratively in a team environment.
- Excellent communication skills both written and verbal.
- Meticulous with strong organizational and documentation skills.
- Possess a high degree of attention to detail.
- Customer service experience (minimum 2 years).
- Computer literate: MS Word & MS Outlook required; MS Excel, EMR system (KIPU), NJSAMS, and IME is a plus. Knowledge of funding/billing sources a plus. Light typing & data entry.
- Valid/insurable driver's license is required.
- Minimum of two (2) years sobriety (if applicable).
- Ability to work independently.
